Follow Up Prescription Weight Loss App

Service Description

If you are ready to have a follow up appointment to track you progress, renew your prescription, and get feedback on your journey to health, this is the appointment for you. If you have not been in our office for prescription weight loss for over 4 months, you must book the intake appointment again prior to doing a follow up.

Cancellation Policy

Appointments canceled will receive a credit for services equal to the paid appointment fee. If a discount was applied to booking the discounted rate will be applicable and rebook-able without an additional fee as long the promotion paid for is still running. In the event the promotion is no longer running, you will be responsible for the remaining payment.
